郵送による課税（非課税）証明書の申請方法(English)　Mail request for issuing taxation certificate

課税（非課税）証明書を郵送により申請する際は、次の4点を同封してお送りください。
When applying for a taxation certificate by mail, Please enclose and send the following four items.

1．申請書　application form

• R4 Application Form for Issuing Taxation Certificate （PDF 81.1KB）

2．手数料（定額小為替）　Commission（TEIGAKU　KOGAWASE）

定額小為替はゆうちょ銀行又は郵便局の貯金窓口でお求めください。小切手、郵便切手又は収入印紙等は、手数料としてお受けすることができません。
Please purchase TEIGAKU KOGAWASE(fixed-amount postal money orders) at Japan Post Bank or post office savings counters. We do not accept checks, postage stamps or revenue stamps as fees.

手数料は1件300円

3．返信用封筒 及び 返信用の郵便切手
Self-addressed envelope and self-addressed postage stamps

郵送により市税の証明書を申請する場合の返信先は、申請者＝納税義務者本人の住所となります。
Return address is the address of the taxpayer. We cannot send it to another person.

4．本人確認書類の写し（マイナンバーカード、運転免許証、パスポートなど）
Copy of identification document (Individual Number Card,residence card, driver's license, passport, etc.)

申請者のマイナンバーカード、在留カード、運転免許証又はパスポートなど、国や地方公共団体の機関が発行した顔写真付きの本人確認書類の写しを必ず同封ください。
Please be sure to enclose a copy of an identity verification document with a face photo issued by a national or local public institution, such as the applicant's　Individual Number Card, residence card, driver's license, or passport.

注意　Attention

• 親族の方からの申請であっても、逗子市において住民登録が同一世帯の親族の方と確認できないときは証明書を発行できないことがあります。
  Even if the application is from a relative, we may not be able to issue a certificate if the resident registration cannot be confirmed with a relative of the same household in Zushi City.
• 逗子市から転出した方について、転出先において同一世帯の親族の方であるかどうか、逗子市では確認できませんので、世帯ごとではなく個人ごとに申請してください。
  Zushi City cannot confirm whether a person who has moved out of Zushi City is a relative of the same household at the new destination, so please apply for each individual, not for each household.
• 郵送で申請をいただいた際は、受領した当日に証明書を発送できるよう努めておりますが、事務処理又は郵便事情など、お手元に届くまで日数を要する場合がありますのでご了承ください。
